Description
Professional Resource Speakers/Assistants to facilitate the conduct of the 3-day, Fleet Management Six (6)Training Schedules as one of the components of Public Utility Vehicle Modernization Program (PUVMP).

Deliverables:

-provision of 1 subject matter expert as resource person
-attendance to consultation meeting/s prior the event
-conduct of pre and post test training assessment of participants

Other services include the following:
1. preparation/submission of activity reports
2. seminar kit (pdf file in USB, handouts and certificates, name tags)
3. content development of training
4. understanding fleet management and operations
5. telemathics,methods of proper monitoring
6. hand-on and work-through using a Fleet Management System and its generating useful management reports
7. sample case study

Training Schedules: March 13-15, 2019 (1st Activity); March 27-29, 2019 (2nd Activity); April 10-12, 2019 (3rd Activity); April 24-26, 2019 (4th Activity); May 15-17, 2019 (5th Activity); May 29-31, 2019 (6th Activity).

Note: Unsealed or unmarked bid envelopes shall be rejected, as the BAC shall assume no responsibility for the misplacement of the contents of the improperly sealed or marked bid. The first envelope shall contain the following technical information/documents:

1. Registration certificate from SEC, DTI for sole proprietorship,
2. Mayor’s/Business permit,
3. Tax clearance per E.O. 398, s.2005 as reviewed and approved by the BIR,
4. Audited financial statement stamped “received” by the BIR, for the preceding calendar year,
5. PhilGEPS Certificate of Registration,
6. Bid Security in the prescribed form, amount and validity period,
7. Certification, under oath, certifying that your consulting firm/company have no pending case(s) against the
Government. (Notarized)
8. Statement of all Ongoing Government and Private Contracts
9. Omnibus Sworn Statement (Notarized)

The second envelope shall contain the financial information/documents
1. Quotation/Financial Proposal.

Reservation Clause: Pursuant to Section 41 of the IRR of R.A. 9184, the procuring entity reserves the right to 1) reject bids, 2) declare a failure of bidding, and 3) not to award the contract without incurring any liability if the funds/allotments for said program/project/activity have been withheld or reduced through no fault of its own.
